Chronic Disease Management
Living with a long-term condition takes a plan, not just prescriptions. We build structured care plans and review them with you through the year.
- Preventive Care
One doctor who knows your whole picture.
Chronic conditions — diabetes, heart disease, asthma, arthritis, osteoporosis, chronic pain — are managed best with continuity. Seeing the same GP means your medications, results and specialist letters all sit in one place, and changes get noticed early.
If you’ve had a condition for six months or more, you may be eligible for a GP chronic condition management plan. It sets out your health goals, the care you’ll receive and who’s involved. A plan can also give you Medicare-rebated visits to allied health providers such as podiatrists, physiotherapists, dietitians and diabetes educators each calendar year.
Plans aren’t set-and-forget. We review yours regularly, adjust treatment as your condition changes, and coordinate with your specialists so nothing falls between the cracks.

- Common Questions
Frequently asked questions
Who is eligible for a chronic condition management plan?
Generally, patients with a condition that has lasted, or is likely to last, six months or more. Your GP will confirm your eligibility and explain what the plan includes.
Does a care plan cover physio or podiatry visits?
A plan can give you access to Medicare rebates for a set number of allied health visits each calendar year, shared across providers. Your GP will refer you to the services that fit your goals.
How often is the plan reviewed?
Typically every three to six months, or sooner if your condition changes. Reviews keep the plan current and renew your allied health referrals.
Can you manage more than one condition in a plan?
Yes. Most patients with chronic conditions have more than one. A single plan covers all of them, which is exactly why continuity with one GP matters.
