Diabetes Management
Good diabetes care is regular, planned and personal. We run your annual cycle of care and keep your numbers — and your risks — in view.
- Preventive Care
Small, steady adjustments beat big corrections
Type 2 diabetes rarely announces itself. It’s picked up through screening, then managed through hundreds of small decisions about food, movement, medication and monitoring. Your GP’s job is to make those decisions easier and catch complications before they take hold.
The annual cycle of care is the backbone: HbA1c at least twice a year, plus regular checks of blood pressure, cholesterol, kidney function, feet, eyes and weight. Each check exists because early treatment of a complication is far more effective than late treatment.
We build a chronic condition management plan around your diabetes, which can include Medicare-rebated visits to diabetes educators, dietitians and podiatrists. If you use insulin or your control is complex, we coordinate with endocrinologists while remaining your home base for day-to-day care.

- Common Questions
Frequently asked questions
How often should I see my GP about diabetes?
Most patients benefit from a review every three to six months, with HbA1c tested at least twice a year. Your GP will set a schedule based on how stable your control is.
What is the annual cycle of care?
A structured yearly set of checks — blood sugar control, blood pressure, cholesterol, kidney function, feet, eyes and weight — designed to catch diabetes complications early, when they’re most treatable.
Can I get help with diet and exercise?
Yes. A chronic condition management plan can fund Medicare-rebated visits to a dietitian, diabetes educator or exercise physiologist. Ask your GP at your next review.
I've been told I have pre-diabetes. What should I do?
Book a consultation. Pre-diabetes is the best possible time to act — modest changes to diet, activity and weight can delay or prevent type 2 diabetes altogether.
